Is the concept of walk-in souls true? Where a soul exchange can happen in the same physical body at the time of a near death experience?

This is completely impossible as the Soul that was using that body has resonance frequency to employ it. Some souls can temporarily posses a body but only if certain circumstances that allow this to happen. Once the body (LIFE) has been assigned to a specific soul it cannot be taken away by another spirit because body will simply – die, and no other soul has the power or capability in any way or shape or form to attach seed itself to the body like a Leach.
